You only need a Fit For Work Note if you are unable to work and are ill for longer than seven calendar days. Your employer will provide you with an SC2 self-certificate form for shorter periods of illness.
When requesting a Fit for Work Note, you will be asked a series of questions by the Receptionist. This is for the doctor’s benefit when completing the Med3 Form.
