Our social prescribing link workers help patients to get the right support. Many different patients will be referred to social prescribing link workers by ether their GP’s/Clinicians or a member of the Patient Services Team for matters such as:
- People with long-term conditions
- Low level mental health issues
- Those who feel lonely or isolated
- Bereavement
Link workers also help people with complex social needs.
Generally, patients speak with a link worker 6 times over a six-month period. The link worker can connect them with activities, groups, and services in their community. This is for practical and emotional support to help them get back on their feet.
