If you change your name, address or telephone number it is very important that you let us know as soon as possible by coming into surgery, telephoning, emailing or writing to us. It is also extremely helpful to have mobile telephone numbers too. If you agree to us having your mobile phone number, please leave your details with reception.
If you move outside the practice area you will need to find a doctor in your new area. There are instructions on what to do when you move to a new area on the back of your NHS medical card. If you have difficulty finding a new Practice, the Health Authority will be able to help you tel no 01904 694700 for patients residing in North Yorkshire and 01482 672109 for patients residing in East Yorkshire.
