The EPaCCs Dataset enables the palliative care service to cooperate with other care providers at the end of a patient’s life. It helps to improve communication between those involved in patient care and ensures that the patient’s wishes are adhered to. The record includes the following data, end of life care status, including patient preferences, social and demographic information such as language, sex, ethnicity, family information, disabilties, general frailty level and advanced care planning. Also included in EPaCCs is a section on the last days of a patient’s life, including anticipatory medicines and death details. Additionally, the dataset lists the patient’s consent for core and additional care information to be shared.
The dataset provides up to date information from the patient’s GP and allows multi -disciplinary teams access to all relevant information to assist in decision making.
