As of April 2014 all patients over 75 are required to have a named doctor who will have overall responsibility for the care and support that Hunmanby Surgery provides them.
You will be informed of your named GP by letter, while this GP will have overall responsibility for the care and support provided by the surgery, you are entitled to see any GP in the Practice. You are also eligible for a free NHS Health check with a Practice Nurse.
This NHS Health Check is a new initiative to improve overall health and wellbeing in the community. Its aim is to improve the care and support given to the over 75’s population. Hunmanby Surgery is very supportive of the scheme and will provide health checks to improve early diagnosis and health screening.
Most patients are already aware of their named doctor, if you are unsure please ask a member of staff. We will try wherever possible to make your appointments with your named Doctor to provide continuity of care.
To book your Health Check please make an appointment at the surgery with a Practice Nurse. If you are unable to visit the practice due to health reasons a home visit can be arranged.
This assessment will provide screening of your health and social care issues based on questions and measurements. Following this you will receive a personalised Care Plan, advice and if necessary onward referral or signposting to appropriate support services.
